On Buddytv.com, they have 8 pictures from the upcoming episode Out With the Old.
After going through them, it seems like Sam is doing ok, despite how he was acting at the end of Repo Man.
However, these could very well be more from the beginning portion of the episode. And as the episode goes on, maybe Sam gets worse and worse to the point where he's bad even during the day--meaning at first Sam's worse time was at night when they were supposed to get some sleep. However, now it's bad all the time where Sam can't hide it even during the day and Dean actually notices.
