Actually, after this clip, I'm of the opinion that Sam has been keeping Dean in the loop about his hallucinations. Dean isn't surprised or even particularly upset that Sam can't get rid of Lucifer. He simply suggests Sam do the hand press. Couple that with Bobby being completely aware that Sam was still hallucinating before he died and I think it confirms that Sam was no hiding his condition. He told them what was going on and updated Dean when things changed.

Maybe Sam has been telling Dean about SOME of his Lucifer hallucianations, but, fromthe casual way he mentioned it in this clip, how is Dean supposed to know just how debilitating these visions are. I dunno, just a thought. Dean loves Sam and wouldn't want him to be in pain, and if he knew just how bad these visions were, he would try to help Sam.....just like he did in that warehouse when Sam was waving the gun around and Dean talked him down, and showed him how reality was different from REALLY being in hell by pressing the cut on his hand. Does anyone REALLY think Dean wants Sam to suffer? Come on!

WANT him to suffer? NO. Be completely unable to figure out how to help Sam? Yes. So he does what little he can, which is tell Sam to do the hand thing and hopes Sam will recover. So far the hand thing did bring Sam relief. Now it no longer does. I'm just saying the "poor Dean he has no warning that Sam is really suffering" attitude (in a review on WFB) seems to be less than accurate.

I like that Sam told Dean what was going on, but I also think that Sam was also kind of downplaying it as well.
My guess would be that Sam doesn't want to harp on it too much because the only reason he let Luci in was to help find Dean. So he doesn't want to put any more guilt or pressure on Dean than Dean already has on himself. In other words, Sam doesn't want Dean to blame himself that Sam's getting worse because of him.

But I'm still confused as to why Sam isn't telling Dean more. Hasn't he realized by now, that when he hides things or lies to Dean, things go south?
Dean's a big boy. He might yell and scream, but that's just Dean letting off steam. Then he'd put his head down and try to figure out a way to help Sam out.

Problem is, there isn't a lot that Dean can do about it, except worry about it. And Sam, at this point, probably feels Dean doesn't need anything else to worry about, that his worry meter is completely filled up. To me, it's not much different than one family member watching another, who has a serious illness and no cure, slowly slip away. And all the first family member can do is just watch.

Agreed. At this point I don't any way for Dean to help Sam with his Lucifer/downed wall issues.
Maybe as new episodes air, we'll get some answers.

I just liked how Sam's problem is out in the open. Dean can't do much for Sam but be there and hear him out. Also in a article is has been said that at the end of episode 16 Dean puts him in the Mental Institution. So obviously Sam's problem worsens as the episode goes on. Also, it was said on twitter that half od Dean and Cas's interactions in episode 17 is about Dean helping and trying to help Sam as much as he can.

He told them what was going on and updated Dean when things changed. Exactly! I don't see how anyone can construe this clip as anything but Sam being completely candid with Dean. But then again the "blame Sam" crowd continues to surprise me. Evidently some can profess loving "both" brothers, and yet look at this clip of Sam telling Dean straight out what is bothering him and say "Why is Sam still such a liar?" I'll never get it.
